Assigning task to supervisor when SLA is breached

Is it possible to have the supervisor get an email when an SLA is breached?

If so:

How do you define a supervisor to the employee?

How do you trigger the SLA > Escalation path?

This should be implemented via a workflow, a custom script that helps you automate the process. There you can build a supervisor-assignee logic, as well an escalation path.
