New ticket button disappeared from helpdesk project

Hi Alisa, yes we do have one active online form, but it is not actively distributed so it is hardly used. All of our ticket come through the email channel. The direct link is:

Gertjan What type of user do you have? If it is a Standard user, please make sure that you have a role in this project that has the Create Issue permission.

Hi Alisa, it is a helpdesk project and my user type is Agent. Under direct roles i have the role Project Admin for this project and under team roles in project i have the role Contributor. Both have the Create Issue permission.

I don't know if this is related, but when i open a console window in my browser and a go to the roles tab on a user page i do get some error messages like the following:

We have already discussed the issue in the support request, but I want to add a note here if anyone stumbles upon the same problem:  it is needed to mark at least one online form as primary.
